Dental Findings:

  • Existing Basal Implants: In mandibular posterior (placed 9 years ago)

  • Conventional Implants: One each in maxillary 2nd and mandibular 4th quadrant

  • Implant Health: No mobility, peri-implantitis, or radiographic compromise

  • Anatomy: Moderate alveolar resorption noted in maxillary premolar and anterior mandibular regions


️ Treatment Goals:

  1. Preserve and incorporate existing stable implants

  2. Minimize surgical insult due to cardiac and anemic profile

  3. Deliver functional esthetics before the wedding deadline


Clinical Execution:

In adherence to Tip 1, the entire protocol centered around minimally invasive techniques:

  • Mandible:

    • Four flapless/minimally elevated endosseous implants placed anteriorly

    • Focus on engaging cortical bone for primary stability

  • Maxilla:

    • Two implants placed in the canine-premolar region, guided by CBCT

    • Implant sites selected based on volumetric bone adequacy

Highlighted Tip: In medically complex cases, leveraging existing osseointegrated implants and choosing flapless or minimal-flap approaches can dramatically reduce healing time and systemic stress.

The surgical procedures were performed under local anesthesia with cardiac-safe vasoconstrictors and antibiotic coverage. No intraoperative complications were noted, and postoperative healing proceeded smoothly.


Outcome:
