Freedom for the Mandible: A Journey with Fabio Savastano and Syed Nabeel into Neuromuscular Dentistry

Chapter 1: Palolem and the Mandible

The golden sands of Palolem Beach glowed like molten glass in the Goan twilight. The tide was rising gently, as though the sea itself wanted to listen. Syed Nabeel—alumnus of RCSI, physiologic dentist, and wanderer—sat at a beach shack nursing a coconut espresso when a familiar silhouette approached.

It was Professor Fabio Savastano, the orthodontist from the University of Naples—the man who built the International College of Neuromuscular Orthodontics and Gnathology (ICNOG) brick by brick, based out of Genoa, Italy. A legend in physiologic circles. Their last meeting had been over two decades ago, back in Calicut, India, when Nabeel was just another disciple trying to make sense of EMG graphs and cranio-cervical mysteries.

They embraced like old comrades, salt in their beards and a glint of shared fire in their eyes.


Chapter 2: Espresso and Occlusion

The waves curled as Nabeel poured strong espresso into two tiny cups. The air was thick with roasted beans and ocean spray.

“Fabio,” he began, “your textbook—it changed the way we talk about muscles. It’s not just occlusion anymore. It’s physiology, posture… it’s life.”

Fabio stirred slowly. “And yet, the mainstream still teaches that centric relation is some divine hinge.”

He looked toward the sea. “But the mandible is not a door. It is a dancer—guided not by force, but by freedom.”

“Freedom for the mandible,” Fabio declared, “is the beginning of healing.”

Nabeel nodded, remembering his first lesson in myocentricity.

“The ideal position,” he replied, “is the one muscles choose when they are at peace. That’s where we begin—not with models, but with EMGs.”


Chapter 3: Beyond the Bite – The Physiologic Awakening

As the beach emptied and stars began to blink, their conversation deepened.

Traditional dentistry emphasized static occlusion—how teeth fit in maximum intercuspation. But Neuromuscular Dentistry (NMD) asked deeper questions:

  • What do the temporomandibular joints (TMJ) want?
  • What are the masseter and temporalis muscles whispering?
  • How does forward head posture redefine the resting bite?

Fabio spoke like a maestro of physiology:

“Muscles do not lie. Dysfunction may hide from articulation paper, but it screams through electromyography.”

They reviewed their sacred tools—not as gadgets, but as windows:

  • EMG – decoding muscle tone, fatigue, hyperactivity
  • TENS – relaxing the stomatognathic system to find the true mandibular rest
  • Jaw Tracking – mapping 3D movements with millimetric clarity
  • Bite Registration in Myocentric Position – capturing the truth, not imposing it

Patients who had suffered for years—neck pain, migraines, even vertigo—began to heal when orthotics restored balance between teeth, muscles, joints, and spine.

Chapter 4: From Teeth to Toes

“Posture,” Fabio said, pointing toward a barefoot yogi on the sand, “is the mandible’s shadow.”

They discussed Posturology—how occlusal planes, cervical curvature, scapular tilt, even foot arches played into the story of dysfunction. Every muscle told a tale. Every misalignment was a clue.

“We’re not just dentists,” Nabeel said. “We are postural architects.”

And Fabio agreed:

“From Camper’s plane to pelvic tilt—dysfunction flows downhill.”

They spoke of steep occlusal planes burdening the upper cervical spine (C1–C3), while flatter planes exerted strain on the lower segments (C5–C7)—an insight often overlooked in mainstream dental discussions.


Chapter 5: A Scanner and a Shift

Nabeel leaned forward, excited.

“Fabio, you must know—the IDS Cologne expo revealed intraoral scanners that can now track mandibular movement in real time. This changes everything.”

Fabio’s eyes lit up. “That’s the bridge between gnathology and data science.”

“Exactly,” Nabeel said. “This is not just a technological upgrade. It’s a paradigm shift. From now on, knowledge of NMD is no longer optional—it’s foundational for every dentist who truly wants to heal.”


Chapter 6: A Proposal Under the Moon

Under a full moon rising over Palolem, Nabeel reached into his bag and pulled out a sketch.

“It’s time, Fabio. Time for a new name. A banner that includes all we believe in—Gnathology, Neuromuscular Dentistry, and Posturology.”

He revealed the emblem: GNPx – Gnathology, NMD, Posturology Xperts.

“Not just a college. A global movement. A stage where we’re not defending science—we’re redefining it.”

Fabio held the paper, eyes moist. “This is what ICNOG always dreamed of becoming.”

They stood by the shoreline, the Goan moon painting silver streaks on the dark water

“I’m planning a new edition,” Fabio finally said.

Instantly, Nabeel replied, “More on dynamic bite simulation, AI-assisted EMG interpretation, and neuroadaptive occlusion — that would be my contribution,” he asserted. “And yes — some new concepts too, which I’ll be publishing in the coming weeks. You might want to include them.”

Fabio nodded with delight. “Then it’s settled.”

Nabeel smiled, placing a hand on his mentor’s shoulder.

“Call me soon,” he said, eyes lit with fire. “Let’s redefine the future—together.”


Final Chapter: Harmony in the Horizon

As they parted ways under a violet Goan sky, Nabeel looked back.

“We used to fix teeth,” he said. “Now we restore harmony.”

Fabio raised his cup.

“And when the mandible is free,” he whispered, “the body follows.”
